EMERGENCIES: This is an office based Consulting Practice. If you have a serious EMERGENCY after hours,

please contact your Family Physician or go to the nearest EMERGENCY ROOM or URGENT CARE CENTER.

QUESTIONS: We'll take your questions during regular office hours and try to respond as quickly as possible.

You may find it easiest to submit your questions by e-mail. We will try our best to monitor incoming e-mail

until 9 o'clock on weekdays, and the e-mail will be checked daily on the weekends.

PRESCRIPTION REFILLS: We'll be happy to help you to maintain your supply of necessary medications. Please

allow adequate time for us to review and refill your prescription, understanding that the process has become

more complicated in recent years. Refills are not "guaranteed", and in the interest of the good practice of

Medicine, prescriptions will not automatically be refilled if you or your family member has not been seen in the

office within 6 months of your request.

FORMS: Forms are a necessary evil. We need your information to comply with insurance regulations, and also

to allow us to run our business in an appropriate fashion. Hate them or not, you've got to update your personal

and Medical information for our records on an interval basis. IF YOU'D LIKE TO SAVE SOME TIME ... EITHER

COMPLETE YOUR FORMS ONLINE, OR DOWNLOAD/PRINT AND COMPLETE THEM AT HOME...AND EITHER

FAX THEM BACK, OR BRING THEM WITH YOU ON THE DAY OF YOUR VISIT. Can't make it much easier!
