The Office...meant to be user friendly, but corrupted by rules, regulations and insurance

industry nonsense. Far from paperless! But seriously folks...We'd love to have you. There

are just a few things you need to know that will help make your visit a positive experience.

Please have realistic expectations...we are truly here to help!

Appointments: It's been over 20 years of office hours, and to our best ability we try to run

on time! I know that your time is as valuable as mine...if not more so! Nobody likes to wait,

and it's really stressful when, as the Doctor, you're running behind. Therefore...we try not to

overschedule, but there are times that difficult medical problems or emergencies make running

late inevitable. Serious cases require thought, explanation, and patience on the part of the

Physician. If your case or that of your family member is one such case, you'll want that extra

time and care, so extend that courtesy to others should the need arise. Please be patient...

we'll let you know it there will be a significant delay, and we'll understand if you have to reschedule

due to time constraints.

ID: IF YOU ARE A NEW PATIENT, OR ARE DUE TO UPDATE YOUR RECORD FILE, YOU WILL BE

ASKED FOR PROOF OF IDENTIFICATION. IF YOU'RE YOU, THIS SHOULD POSE NO PROBLEMS. IF,

PERHAPS, YOU'RE AN EXACT DUPLICATE OF YOURSELF WHO'S BEEN INSERTED INTO SOCIETY BY

AN ALIEN RACE, YOU SHOULD STILL BEAR A REASONABLE RESEMBLANCE TO YOUR DRIVERS LICENSE

PHOTO, AND THERE SHOULD STILL BE NO PROBLEM. IF YOU PLAN TO MAKE A SCENE ABOUT THIS

REQUEST IN THE WAITING ROOM ON THE DAY OF YOUR VISIT, OR ARE JUST COMING IN FOR AN

ARGUMENT, PLEASE MAKE A SEPARATE APPOINTMENT FOR THIS PURPOSE, AND PLEASE MAKE

SURE TO PREPAY FOR THAT VISIT...(JUST MAKE BELIEVE YOU'RE GOING TO THE AIRPORT!!! )

Forms: In order to properly maintain your medical records, we need you to provide us with some

basic information regarding your medical history. We also need accurate insurance information.

Please be patient with us? We know you've been filling out the same forms at other Physicians offices!

We are, however, REQUIRED by all insurers to maintain this information.
